Approaches to a child with fast breathing APPROACH TO A CHILD WITH FAST BREATHING Fast breathing is the most common presentation in children visiting a hospital emergency. These children have the respiratory rate more than the normal upper limit for that age group; (see table 1), with or without increased work of breathing in the form of chest indrawing, nasal flaring and head nodding. It may also be associated with stridor or wheeze suggestive of upper and lower airway obstruction respectively. There is a need of urgent assessment of airway patency and breathing when a child with fast breathing is first evaluated. Stabilization of vital parameters may require intubation, oronasal suctioning, use of oxygen by hood/nasal prongs, intravenous fluid boluses, correction of hypoglycaemia, nebulization with bronchodilator, intercostal tube drainage, correction of hyperthermia/ hypothermia etc. Such initial treatment coupled with a thorough history, physical examination and relevant investigations, is followed by establishing a provisional diagnosis and instituting approp riate empirical treatment in the emergency ward itself. Table 1: The upper limits of respiratory rate defined by the WHO Etiology of fast breathing: Fast breathing may not always result from a lung disease. It may be physiological e.g., exercise induced, or pathological due to pulmonary or non-pulmonary causes (table 2) Table 2: Causes of fast breathing in children Clinical Features: A child with fast breathing be may have increased work of breathing (suggested by use of accessory muscles), cyanosis and lethargy or altered sensorium. Alteration in sensorium (in the form of irritability, agitation, lethargy or coma) indicates brain hypoxia and is one of the earliest indicators of impending respiratory failure. While fast breathing is commonly associated with respiratory diseases, it may also occur with fever, crying or metabolic acidosis. However, normal or decreased respiratory rate may be more ominous if it is associated with severe retractions (paradoxical breathing), cyanosis, grunting or altered sensorium. Central cyanosis is a late sign but may not be detected in presence of severe pallor (low Hb) and dark skin colour. Stridor is a harsh inspiratory sound that indicates upper airway obstruction. Grunt is a loud noise produced by a forceful expiration against a closed glottis. Grunt and wheeze (a musical sound) are suggestive of lower airway obstruction. A complete history should reveal the onset, duration, progression of dyspnea, the aggravating and relieving factors as well as the associated symptoms like fever, cough, sore throat, chest pain, choking episodes, accidental ingestion of poisons etc. (table 3) Table 3: Symptom based diagnostic clues Clinical pearls: Investigations: Laboratory investigations help to confirm the diagnosis but the immediate management of a patient should not be delayed pending the reports of the investigations. Use of non-invasive devices such as pulse oximeter and ET CO2 detector (fitted in the ventilator) lessen the need for repeated invasive tests for monitoring of the child. Table 4 shows the relevant investigations to ascertain the cause of respiratory distress in a child. Table 4: Laboratory investiagations Treatment: The management of a child with fast breathing includes supportive treatment in the form of stabilization of vital parameters i.e. temperature, airway, breathing and circulation followed by definitive treatment by instituting appropriate respiratory support, antibiotics, chest tube drainage, decongestive measures etc. Acute onset of fast breathing, esp following choking, and stridor indicate foreign body, and warrants prompt bronchoscopic search and removal of foreign body. Algorithmic approach to management of fast breathing: Conclusion: It is essential to promptly triage children with impending respiratory failure and quickly institute supportive management, simultaneously searching for the etiology and planning a definitive treatment. The above mentioned approach will improve the outcome of children, especially the under-five ones, in whom respiratory infections contribute to the highest number of mortalities. Do you A: Answer â€Å"Yes† and take the food or B: Answer â€Å"No† and tell him/her that you only paid for a shake? Yes this really did happen to me. What did I do? Believe it or not, I did the honest thing and gave the food back. But the real question is what would a majority of American’s youth do in a similar situation. There are two potentially dishonest acts in this situation, lying and stealing. In my opinion most high school kids are not above lying and/or stealing. The reason this is true is simply that many of today’s parents are not above lying and/or stealing. So after observing people that hold a high place of respect do things unmoral like this, their conscience says to itself, â€Å"Okay, lying and/or stealing is not really that bad, and is acceptable to my parents.† A majority of Americans youth today are morally confused, due to the un-honorable actions of their parents.   Ã‚  Ã‚  Ã‚  Ã‚  1984, by George Orwell, shows an example of un-honorable actions of parents, rubbing off on their children. About 99% of the population in this book, were uncaring robots. They would turn their best friend into the â€Å"thought police† at the drop of a hat. The children of 1984, were even worse because they would turn their own parents into the â€Å"thought police†, with no sense of shame, and they actually felt proud about their actions. This is a perfect example of the unmoral actions of the parents rubbing off on their children. This could be the fate of our country if we don’t take parenting more seriously. Well we can start by thinking about what we do before we do it and putting ourselves into the other person’s shoes. Jack grew as skilled on horseback as any Hillman, for all that he had come to it so late; and Mathin took him to his home village, where he learned how the Hills trained their young horses. He was good at this too, and Mathin's family liked him, but always he found himself returning to the stone City, where Corlath seemed more content to stay since Harry now stayed with him. And the year that young Tor Mathin was two years old, Jack was called to a banquet in the Great Hall, where he had attended many banquets before, and to his own amazement he was made a queen's Rider, to sit with the fifteen king's Riders, for Corlath had made no more since the war with the North. Gonturan, which Jack had held once before on a mountaintop, lightly and kindly drank three drops of his blood, while he stared at the cut and for once had nothing to say. â€Å"We Outlanders must stick together,† said Harry, smiling. Jack looked up at once and shook his head. â€Å"No – we who love the Hills must stick together.† The year after Jack was made a Rider, Harry bore another child, and this one was a daughter, and she had red hair and blue eyes, and a wry whimsical smile even in her cradle. â€Å"You're calling her Aerin, of course,† said Jack, tickling her with the end of his sash while she giggled and clutched at it. â€Å"I'm calling her Aerin Amelia, and Forloy and Innath and Mathin and I are riding west as soon as she's six months old, to invite Sir Charles and Lady Amelia to the Naming, here in the City. Will you come with us?† Harry was holding her baby, and as Jack, startled, stopped looking at her and instead looked up at her mother, Aerin grabbed the sash and stuffed as much of it as would fit into her mouth. â€Å"Yes, of course I'll come. There were traveling-bundles hanging from his saddle, and Harry's face lit up and she said: â€Å"Oh, you are coming with us after all.† And Corlath sighed, and reached over Sungold's withers to take her hand and said, â€Å"Yes, I'm coming. I don't want to, you understand. Perhaps you should just think that I cannot bear to be parted from you for so many days; which is true enough.† â€Å"I don't care,† said Harry. Corlath looked at her and smiled in spite of himself. â€Å"Perhaps you are right, my heart. I am inclined to forget that there is still some Outlander blood in your veins; and perhaps this mad scheme of yours will work.† The six of them stopped and set up camp where a much bigger traveling camp had stopped several years before, to wait upon another visit to the Outlander town. I'm glad you didn't quite forget us. You don't have to name the baby after me, you know – † her voice was muffled, because it was buried in Harry's shoulder – â€Å"but if you meant the invitation, I shall certainly come. And Charles too.† Harry looked up, and Sir Charles was ponderously dismounting. Lady Amelia let her go, and Sir Charles said nothing as he embraced her in his turn; and his silence she thought was a bad omen till she looked into his face and saw the tears in his eyes. He snuffled through his mustache once or twice, and then his eyes opened wider as they looked over Harry's shoulder, and she heard Jack's voice saying: â€Å"Good to see you again, old friend.† The meeting between Sir Charles and Corlath was a trifle constrained. Sir Charles, forgetting himself in an attempt to get off on the right foot this time around, put out his hand; and Corlath looked at it, and looked at Sir Charles, and Harry gritted her teeth; and then Corlath seemed to remember a description, from her perhaps, or from Jack, of this curious Outlander ritual; and he put out his hand, tentatively, and Sir Charles shook it heartily. After that things went more or less smoothly; and Sir Charles spoke the Hill tongue, not nearly so badly as Corlath had privately been expecting – he's been practicing, the Hill-king thought in surprise, and felt almost warm toward him – and Corlath spoke Homelander, and Sir Charles tactfully refrained from remarking on how fluently he knew it. Sir Charles wanted to insist that they all return to the Residency while he and Lady Amelia packed up for their journey, and Jack could see how he was trying to restrain himself, so he spoke to Harry and Harry spoke to Corlath. And Corlath eyed his wife and thought dark thoughts; but eight riders rode back toward Istan together. And so diplomatic relations between Outlander and Damarian began, for the first time since the Outlanders had come over the sea and seized as much as they could. Jack discovered that Sir Charles had taken his letter, written while Harry and Senay and Terim and Narknon lay asleep in his bedroom, very seriously indeed; and had, in fact, put his own career in jeopardy by insisting that the colonel of the General Mundy had not gone desert-mad at last, but had answered a real threat to Outlander security in the only way he could. It was because of Sir Charles' efforts that Jack himself and the men who had gone with him were honorably listed in the military rolls as missing in action at the Border and presumed deceased. So when he returned from the Naming, leaving Lady Amelia behind for an extended visit with her name-child in the stone City, and began writing dispatches about the time being ripe for the opening of formal diplomacy between the Homeland and Damar – for so he called it – he was permitted to pursue the role he had chosen. It is true that only he and Lady Amelia were ever invited to the City in the Hills; but specially chosen Damarians did begin regularly to visit Istan, and eventually the cities in the south; and to exchange gifts, and speeches of good will, and to receive official administrative notice, even from the Queen and her Council, over the sea in the Homeland. And Harry and Corlath attended to their administrative duties as earnestly as they had to, but no more; and much of their time they spent wandering alone together through the City, or across the plains before the City; or they rode to Mathin's village, or Innath's; and as often as they could they slipped away north through the Hills to Luthe's valley. After years of discussion and over half a century of waiting, the United States has finally honored the Americans who helped fight World War II with a memorial. The World War II Memorial, which opened to the public on April 29, 2004, is located at what was once the Rainbow Pool, centered between the Lincoln Memorial and the Washington Monument. The Idea The idea of a WWII Memorial in Washington DC was first brought to Congress in 1987 by ​Representative Marcy Kaptur (D-Ohio) at the suggestion of World War II veteran Roger Dubin. After several years of discussion and additional legislation, President Bill Clinton signed Public Law 103-32 on May 25, 1993, authorizing the American Battle Monuments Commission (ABMC) to establish a WWII Memorial. In 1995, seven sites were discussed for the Memorial. Though the Constitution Gardens site was initially selected, it was later decided that it was not a prominent enough location for a memorial commemorating such an important event in history. After more research and discussion, the Rainbow Pool site was agreed upon. The Design In 1996, a two-stage design competition was opened. Out of 400 preliminary designs entered, six were chosen to compete in the second stage which required review by a design jury. After careful review, the design by architect Friedrich St. Florian was chosen. St. Florians design consisted of the Rainbow Pool (lowered and reduced in size by 15 percent) in a sunken plaza, surrounded in a circular pattern with 56 pillars (each 17-feet-high) which represent the unity of the U.S. states and territories during the war. Visitors would enter the sunken plaza on ramps which will pass by two giant arches (each 41-feet tall) that represent the two fronts of the war. Inside, there would be a Freedom Wall covered with 4,000 gold stars, each representing 100 Americans that died during World War II. A sculpture by Ray Kasky would be placed in the middle of the Rainbow Pool and two fountains would send water more than 30-feet into the air. The Funds Needed The 7.4 acre WWII Memorial was estimated to cost a total of $175 million to build, which includes future estimated maintenance fees. World War II veteran and Senator Bob Dole and Fed-Ex founder Frederick W. Smith were the national co-chairmen of the fund-raising campaign. Amazingly, approximately $195 million was collected, almost all from private contributions. Controversy Unfortunately, there has been some criticism over the Memorial. Though the critics were in favor of a WWII Memorial, they strongly opposed its location. The critics formed the National Coalition to Save Our Mall in order to stop the construction of the Memorial at the Rainbow Pool. They argued that placing the Memorial at that location destroys the historic view between the Lincoln Memorial and the Washington Monument. Construction On November 11, 2000, Veterans Day, there was a ground-breaking ceremony held on the National Mall. Senator Bob Dole, actor Tom Hanks, President Bill Clinton, a 101-year-old mother of a fallen soldier, and 7,000 others attended the ceremony. War-era songs were played by the U.S. Army Band, clips of war-time footage were shown on large screens, and a computerized 3-D walkthrough of the Memorial was available. Actual construction of the Memorial began in September 2001. Constructed of mostly bronze and granite, the construction took three years to complete. On Thursday, April 29, 2004, the site first opened to the public. The formal dedication of the Memorial was held on May 29, 2004. The World War II Memorial honors the 16 million men and women who served in the U.S. armed services, the 400,000 who died in the war, and the millions of Americans who supported the war on the home front.
